Empathetic Dementia Assistance
I support individuals living with dementia and their families through compassionate care planning, education, and ongoing guidance tailored to each stage of memory loss. I help address safety concerns, communication challenges, behavioral changes, daily care needs, and caregiver stress while coordinating appropriate resources, support services, and living arrangements that promote quality of life, well-being, and dignity.

Future Planning
I help individuals and families prepare for future care needs by developing proactive strategies that support long-term health, safety, and quality of life. I provide consistent communication, coordination, and oversight for families caring for loved ones from a distance. I monitor safety, health, and daily needs, coordinate local services, and serve as a reliable on-the-ground presence. I also provide regular updates, advocate on your behalf, and help ensure care remains consistent, responsive, and aligned with evolving needs and circumstances.

Rehabilitation Care Support & Discharge Planning
I coordinate smooth transitions from hospital to home or rehabilitation settings by collaborating with healthcare teams, reviewing discharge instructions, and organizing follow-up care. I help manage medications, therapies, and treatment plans, arrange in-home support services, and ensure clear communication with families. My goal is to support recovery, reduce complications, and minimize hospital readmission risk through proactive, continuous care coordination.

Telephone/Video Consultations
I offer virtual consultations to help individuals and families address concerns, explore care options, and determine whether ongoing care management services are needed. During these sessions, I provide guidance, answer questions, review challenges, and discuss practical strategies and resources.
